This is a tough one, since there were many scenes (other than traditional stunt work) where they used a double since Ford was out for a long period due to his back injury. Spielberg says on one of the video bonus dealies that he shot everything he possibly could without Ford once the injury happened and he knew Ford would be out of action for a while.
Now, as far as jacket related stunts in ToD, I can't think of many. The whip swing over the lava pit in the main temple is one. The raft sequence would have been another. Beyond that it seems Indy was jacketless for the bulk of the film, especially the big stunt sequences.

Not a stunt but when Indy walks back into the village at the end of the movie down the hillside. I think that's a stand in.

The 65-year-old suffered a chronic back injury while riding an elephant in Sri Lanka during filming on Indiana Jones And The Temple Of Doom in 1984.
But Ford failed to recover from his injury and was forced to seek medical treatment after returning to the U.K.
And he was surprised when help was offered by British soccer team Manchester United.

I think he suffered a herniated disk in his back supposedly started by straddling an elephant in Sri Lanka and later ruptured while doing a flip during his Pankot bedroom fight scene filmed on set in England. I've heard he was out for 6 weeks. He's supposedly never had a problem with it since. The pain for that injury is supposed to be excruciating and yet he continued filming scenes and resting on a hospital bed that was brought in until Lucas put a halt to it. Harrison gives a lot credence to the phrase Ford Tough. I think there is also some mention of this in the ToD extras on one of the DVD's.

Yeah, that's exactly what I heard. 90% of everything I've ever heard about it came from watching the bonus features on the late-90's VHS release and the two DVD releases, though the 2003 DVD bonus feature is the one I remember best. But it's been pretty well documented otherwise. Doug, hadn't you ever heard that story about Ford's injury before?

I think I've mentioned this before but it's fun. Watch when Pat Roach, as the big Thuggee baddie, picks up Indy and throws him in the mine car.... that ain't Ford it's a big cloth dummy! .
